GETTING STARTED

Fees and Insurance

 

My fee is $130 per 50 minutes office session. Online sessions are available.


Payment

 

Payment is due at the time of our session. I accept cash, check and Paypal, to email address nichole@nicholelorenz.com as forms of payment.

I'm currently accepting Blue Cross Blue Shield, and Blue Care Network. For other plans, you can call your insurance company to determine if they will reimburse you for out-of-network care. If you are using your insurance benefits to cover the cost of therapy, please contact your insurance company to clarify your co-pay or co-insurance for each session. Co-pay and deductible payments are due at the beginning of each session. Bring a copy of your insurance card –both front and back– with you to your appointment.

Intake Forms

 

Please download, print, and fill out your intake form and bring it to your first appointment. If you don't have a printer, or access to one please plan to arrive 40 minutes early to fill out paperwork in the office. 


Cancellation Policy

 

If you are unable to attend a session, please cancel at least 24 hours beforehand. Otherwise, you may be charged the full fee for our meeting.
